I hereby am cancelling my 54 Club membership at Orange Lake as of the expiration date on file. I understand that if this form is submitted inside the 30 day window of cancellation that my membership may not be cancelled and I will be billed for the following year. Orange Lake Golf management may reach out to me discussing cancellations made inside 30 days. Orange Lake Golf management will email back a confirmation of cancellation once the membership cancellation is made within the guidelines of the signed agreement made when beginning the membership.
